Birth Certificates

As of February of 2010, the Department of Juvenile Justice (DJJ) is authorized to request a certified copy of a DJJ-involved youth's birth certificate if unavailable from other sources as a result of a Memorandum of Understanding with the Department of Health.

Completion of the top half of the application is required by the youth, with either the Chief Probation Officer, FJJF Foundation or DJJ Education Director completing the second half as requestor. The Birth Certificate will be mailed back to the requesting party for provision to the youth. A $9 fee is required. The Youth Investment Award application may be used to request the $9 per application for each individual applicant.

Questions may also be directed to Ms. Caroline Ray at (850) 717-2705 or Caroline.Ray@djj.state.fl.us.
